Does LASIK hurt during surgery, what about as I am healing?

During the LASIK procedure, there is a slight bit of pressure when the flap is created, according to Dr. Sandy T. Feldman. However, the rest of the procedure is very simple and involves looking at a flashing red light. During the healing process, it is normal to experience some irritation and a gritty feeling in the eyes.

How do contacts work for monovision?

Monovision contact lenses essentially achieve similar visual results as laser vision correction would. In most cases, the dominant eye would be corrected for distance and the non-dominant eye would be slightly under-corrected for near vision. People who might consider monovision are usually around or above the age of 40, where the loss of near vision, or presbyopia has progressed.

Is Visine good for your eyes?

Visine "gets the red out." The problem, according to Dr. Sandy T. Feldman, is that it contains low levels of alcohol, which may worsen the cause of the redness. Additionally, it contains an agent to constrict the blood vessels, which when it wears off, just get red again.

My eyes get itchy during the spring, what can I do to make them feel better?

According to Dr. Sandy T. Feldman, you should consult your eye doctor regarding the possibility of allergies if your eyes are itchy in the spring. Allergies also can play a role in a patients eyes feeling itchy before and after laser vision correction. Your doctor may wish to prescribe oral medication and in some instances "allergy relief" eye drops.

What is Hyperopia?

Hyperopia is the medical term for not being able to see near images. If you have this problem, your cornea (the clear part of your eye covering the pupil) is flat. When light enters the cornea of someone with hyperopia, the light focuses behind the retina instead of on it. The eye, therefore, cannot properly turn the light into images and the result is blurred vision.

CK (conductive kearatoplasty) is one procedure used to treat hyperopia. The CK radio waves alter the shape of the cornea. The application of the CK waves is rather quick and painless. Many patients describe the feeling as a slight pressure on the eye. CK is also used to treat presbyopia, which is an age-related disorder where the eye becomes unable to focus for close up activities like reading.

What kind of eye drops do I need to use after LASIK?

Will my eyes feel scratchy after LASIK?

Most patient who have the LASIK procedure do not note much discomfort after their procedure. According to Dr. Sandy T. Feldman, it is important after your procedure to take the indicated medicine to alleviate discomfort, as well as going home and sleeping for approximately four hours.

Why can’t highly nearsighted patients have LASIK?

In some cases, patients who have a high amount of nearsightedness may not have sufficient corneal thickness to create a flap and then apply the laser which also decreases corneal thickness as it reshapes the cornea itself. You should consult with our doctor in order to learn more.

Sometimes, doctors can perform surface laser vision correction procedures such as PRK [photorefractive keratectomy], ASA [advanced surface ablation], LASEK or epi-LASIK or implantable lenses such as Visian and Verisyse to correct vision in highly nearsighted individuals.

Conductive Keratoplasty

Conductive keratoplasty (CK) is a minimally invasive procedure that uses low energy radio frequency light to treat farsightedness and presbyopia. CK is well-suited for patients who are 45 or older and patients who are not good candidates for LASIK.

With CK, one eye is adjusted for near vision and the other eye is adjusted for distance vision. During CK, our surgeon uses the light beams to increase the curvature of the cornea, which improves vision almost immediately.

There is no cutting of the eye with CK; CK has a very high safety record and is used on patients who are not good candidates for LASIK. The effects of CK may not be permanent; repeat treatment may be necessary in a few years.

I have heard that I need to have my contact lenses out for a period of time before LASIK, how long?

According to Dr. Sandy T. Feldman, the duration in which a patient should refrain from wearing contacts is determined by the type of lens and how a patient wears them. What type of laser is used for LASIK?

An excimer laser is an ultraviolet laser which contours the front surface of the eye, known as the cornea, is used during the LASIK procedure. We use the latest vision correction technology including VISX Advanced CustomVue with Iris Registration. Iris registration enables proper alignment of the wavefront guided procedure.

Why is Intralase better than traditional LASIK?

The Intralase technologically is an advanced method of creating a thin corneal flap in LASIK. Our prior methods in traditional LASIK used a semi-automated mechanical device known as a microkeratome.

To create the flap, the Intralase laser precisely positions a layer of bubbles just beneath the surface of your eye, and by doing so, it generates a smooth even surface after your flap is lifted. The gas bubbles separate the tissue to fashion the flap. With the IntraLase Method, a blade never touches your eye. The Intralase is also computer controlled enabling better precision to control the depth, size and shape of the flap.

LASIK for Aviators

Approximately 145 million Americans, which make up approximately 54% of the population, are dependent upon corrective lenses to achieve a vision satisfactory for their daily needs. These individuals have refractive conditions such as myopia, farsighedness, and astigmatism that keep light rays from being focused as a clear, single image on the retina.

It is crucial that pilots have superb vision since visual cues supply about 80% of all flight information. Pilots must detect and identify airborne traffic and any hazards that may be on runways and taxiways. Flight manifests, charts, and instruments in the cockpits need to be clearly seen to ensure that proper flight procedures are followed. In an aviation environment where visual conditions are not always the best, an aviator's choice of refractive correction becomes a serious consideration.

Laser assisted in situ keratomileusis (LASIK) is a method of vision correction performed by ophthalmic surgeons for the treatment of myopia. LASIK is performed using two FDA-approved tools: the microkeratome and the excimer laser. The microkeratome is a planing scalpel that has been used for 30 years to perform other types of refractive surgery.

There can be complications with some LASIK patients. Some patients experience mild irritation, sensitivity to bright light, and tearing for couple of days following LASIK. If your eyes get infected, corneal scarring may occur and may reduce the clarity of your vision. After LASIK, patients should be cautioned to avoid vigorous rubbing of the eyes and contact sports as it can take up to 6 months for the corneal flap to completely re-adhere.

What is PRK?

PRK (photorefractive keratectomy) is a surgical procedure that treats nearsightedness, mile to moderate farsightedness and astigmatism by flattening the center of the cornea with beams of ultraviolet light. The computer used in the procedure generates the amount of laser light necessary to remove a calculated amount of corneal tissue (based on individual refraction).

Our eye surgeons use an excimer laser to remove tissue from the surface of the cornea, allowing it to focus light correctly. If you are nearsighted, we will perform PRK to flatten the too-steep cornea. If you are farsighted, the doctor will perform PRK to create a steeper cornea. Three days following PRK, your vision will begin to improve and then will steadily improve after that.

Is IntraLase LASIK better than traditional LASIK?

The IntraLase is a femtosecond laser which is used to create fine gas bubbles to make the LASIK flap, rather than the older instrument known as the microkeratome. The microkeratome is a semiautomated mechanical instrument which was used for about ten years to create a flap. The Intralase is one of the components needed to perform the most up to date LASIK procedure, known as iLASIK.

Do I need to worry about hazy vision from LASIK?

LASIK is a procedure which often produces instantaneous results. However, patients tend to forget that there is a natural healing process following surgery. Some patients report that the vision may be hazy or fuzzy and feel like wearing a dirty contact lens. Such vision can result from a number of different causes including a small residual prescription of the eye, dryness or swelling of the eye. How Wavefront Diagnosis Works

Wavefront-guided LASIK diagnoses the eye’s refractive error using a system that gathers extensive information about your eyes. Your treatment is based on the detailed information the system collects, giving you much more precise vision correction.

How it Works

While you focus on a target through the eyepiece, a straight laser beam is directed into your eye. When that light reflects back from your eye to the system’s sensor, it is no longer parallel or straight; it has changed at the front. The change expresses the eye’s refractive error. The deviations from the straight light are caused by tiny irregularities in the eye. The light has reflected back through the lens and cornea to the Wavefront system’s sensor, and by the time it arrives, it has been distorted by the eye’s irregularities.

What if I move during my LASIK procedure?

Moving or blinking during your LASIK procedure is a common concern. In order to keep the eye from blinking, we use an instrument to hold your eyelids in place during the procedure. In addition, there are quite a few safety mechanisms on the laser. Even if you did move, the laser will automatically stop.

For a few hours after their procedure, patients may experience watery, teary eyes, as well as a foreign body sensation in the eyes. This is why Dr. Sandy T. What if I have dry eyes after LASIK?

After LASIK or PRK surgery, your eyes may be dry. Dry eyes are a temporary complication which can be treated with lubricating eye drops or sometimes with inserts or “plugs”. Dry eyes usually clear up within a few months after your procedure. The key to treating dry eyes is to lubricate often, usually several times a day.

Monovision, the solution for old eyes

While LASIK results are stable, our eyes change as we age and lose the ability to see up close. This is known as presbyopia and causes most of us with perfect vision in our 40’s to have difficulty seeing the menu in dimly lit restaurants or putting on make up. A solution might be laser vision correction with monovision.

With monovision, Dr. Sandy T. Feldman treats the dominant eye for distance and the nondominant for reading. While this sounds odd to most people, it is a great option for those of us with “old” eyes. Diabetes-Glaucoma Link?

Recent medical reports have indicated that diabetes may lead to glaucoma in addition to diabetic retinopathy. A recent study in “Ophthalmology” found that women with diabetes have about a 70% increased risk of developing the most common form of glaucoma (open angle glaucoma) compared to women who do not have the disease.

Currently, open angle glaucoma affects approximately 2 million people in the United States and is one of the leading causes of blindness. However, to date, the evidence is not conclusive.

Glaucoma occurs when there’s a gradual increase in the normal fluid pressure inside the eyes. Glaucoma causes damage to the optic nerve which can result in vision loss and eventual blindness. Approximately 50 million Americans are at risk for vision loss from glaucoma.

One way that diabetes can possibly cause glaucoma is by causing elevation in pressure within the eye. Also, diabetes could increase the susceptibility of the optic nerve to damage.

What is astigmatism of the eye?

When the cornea or front surface of the eye is an oval shape instead of the normal round shape it is known as astigmatism. This condition causes blurry, ghosting and double vision. Astigmatism can be treated with laser vision correction.

Can I wear eye makeup after LASIK surgery?

Dr. Sandy T. Feldman advises her patients to not wear eye makeup for about a week after LASIK surgery. It is important that when patient begin to wear makeup again that they use clean brushes, new mascara and remove their makeup gently with liquid make up remover.

CustomVue LASIK uses WaveScan® technology in order to deliver a safer and more accurate procedure. The WaveScan works by creating a map of all the imperfections in your vision so that a custom and personalized treatment plan can be developed. Lasers in Eye Surgery

In the United States, there are four wavefront responsive laser manufacturers. The trade names are CustomCornea using the Alcon LADARVision laser, CustomVue using the Visx S4 laser, and Zyoptix using the Bausch & Lomb Technolas 217z laser. These three have the ability to create an ablation customized from an individual patient's wavefront evaluation.

The Wave Light Allegretto laser used wavefront derived data in the laboratory to optimize its laser ablation pattern but does not use an individual patient's wavefront diagnosis to create a customized ablation pattern. CustomVue, CustomCornea, and Zyoptix are all customized to the individual's eye. Visx, Alcon, and B&L refer to their systems as "custom wavefront.” Allegretto is referred to as "wavefront optimized.”

Alternatives to LASIK surgery

LASIK is a great procedure and can treat nearsightedness, farsightedness and astigmatism. However, LASIK may not be the best procedure for patients with thin corneas. For those patients, PRK, or photorefractive keratectomy, might be a better alternative.

There are also other procedures, known as implantable lens [ICL, Visian] or phakic intraocular lens [Verisyse], in which a lens is implanted inside the eye to treat highly nearsighted individuals.
